RKI Primary Care At Home is a nurse practitioner–owned and operated practice founded by Uche Ezedi, a board-certified Family Nurse Practitioner.
Uche brings over three years of clinical experience across outpatient elder placement clinics, home health, and correctional healthcare settings. She has provided comprehensive, patient-centered care both in clinic environments and directly in patients’ homes, allowing her to develop a well-rounded and adaptable approach to primary care.
Her clinical focus emphasizes accessibility, continuity of care, and meeting patients where they are—ensuring that each individual receives personalized, high-quality healthcare in a setting that is comfortable and convenient.
